Pfizer Recalls Birth Control Pills After Mixup

Drugmaker says 1 million packets may contain inadequate doses of the active ingredients needed to prevent pregnancy.

Pfizer is recalling 1 million packets of birth control because some might not contain a high-enough dose of hormone-based drugs to prevent pregnancy.

In a statement Wednesday night, the drugmaker said the recall affects 14 lots of Lo/Ovral-28 tablets and 14 lots of Norgestrel and Ethinyl Estradiol tablets. Essentially, some of the packets contain too few pills with the active ingredient needed to prevent pregnancy, and some packets contain too many.

Oral contraceptive packets typically contain both active and inactive pills in order to regulate the menstrual cycle: Normal pill packages should have 21 "active" pills and 7 sugar pills with no active ingredients. If the count is off, those taking the pills as a form of birth control may no longer be protected against pregnancy.

For those who think they might have an affected packet, check the lot number here.

The pill packets in question were distributed to warehouses, clinics and retail pharmacies across the country, and have expiration dates between July 31, 2013 and March 31, 2014. Recalled packets should be returned to a pharmacy, and users should notify their doctors. Pfizer notes that taking pills from a recalled packet doesn't pose a health risk, but recommends the adaption of nonhormonal birth control "immediately" for those with recalled packets.

Germany mulls stricter labeling rules for regional foods

Are the apples in locally branded apple juice always local?

German consumers like their local produce. But labels are often misleading. Germany's Consumer Protection Ministry believes there should be labeling rules for regionally produced and processed foods.

Names aren't always what you think they are, especially when it comes to regional German foods.

"Black Forest Crème with sheep's cheese," for instance, is the name of a cream cheese produced by the German company "Schwarzwaldmilch" or "Black Forest Milk." With a label like that, most consumers would think they were buying a product made from the milk of sheep grazing on pastures somewhere in the Black Forest, right?

Wrong: The cream cheese has little to do with the Black Forest. The product contains only 25 percent sheep's milk - and it comes from Greece, according to a complaint filed on the consumer protection website "Lebensmittelklarheit.de."

Details about regional origins

The Internet platform, launched earlier this year by the German ministry for Consumer Protection, provides a forum for consumers who feel misled to express their view and also informs them about labeling rules.

Germany's Consumer Minister Ilse Aigner seeks clear criteria for local produceIn a survey this summer, 80 percent of consumers said they thought details about the regional origin of a product were important.

While the European Union has so-called "geographical protection" labeling rules, Germany's Consumer Protection Minister Ilse Aigner told Dow Jones that these laws are "concerned with protecting traditional recipes" and not necessarily regionally raised and produced food products.

Aigner's spokeswoman Mareike Enderle told Deutsche Welle that one of the difficulties lies in defining the term "regional."

"We are currently working on coming up with a framework, together with the consumer ministries in the states or Bundesländer, as well as with producer associations, retail industry and consumer rights organizations," she said.

That could prove to be a challenge. The different stakeholders also have different views on what constitutes a regional product.

Difficult issues

For some consumer activists, all ingredients should originate in a given region. For retailers, on the other hand, a certain percentage of local ingredients would suffice for the whole product to be labeled regional.

Rules on what constitutes organic food are clearFood experts point to difficult issues, such as whether regions should have a defined radius or whether the term regional should refer to where a product is manufactured or where the ingredients are from.

"What counts is that we have one accepted definition," Hedi Grunewald, a food expert at Lower Saxony's consumer advice center, told Deutsche Welle. "I would say a product can bear the label 'regional' if it is produced in a certain region with products grown elsewhere. In that case, it only has to be made clear that production is regional, while the ingredients are not."

With the German "Biosiegel," the rules are fairly clear. Since 1993, terms such as "organic" and "bio" have been legally protected. Foods bearing the official "Biosiegel" label for organic produce must have up to 95 percent organic ingredients.

In 2010, the EU also introduced a label for organic produce, requiring all foods bearing the label to consist of at least 95 percent organically produced ingredients.

'Geograpical protection'

The European Union also has legislation that offers producers of regional specialty foods so-called "geographical protection" for their products.

For mountain cheese from the Allgäu region in southern Germany, for instance, raw materials used in the cheese must originate in the Allgäu, and the production process must take place in the region as well. Under EU legislation, "Allgäuer Bergkäse" has what is called PDO status, or "protected designation of origin."

But the situation is different with products like "Black Forest Ham" or "Dresden Christmas Stollen." Their status is one of "protected geographical indication," meaning that only one step of the production process must take place in the indicated region, while the ingredients may come from other regions or countries. So it is entirely possible that Danish pigs supply ham for the Black Forest Ham.

During the international foods fair "Grüne Woche" early next year, Germany's consumer protection ministry intends to go public with the results of the discussion on how regional labeling could work.

For consumer rights advocate Grunewald, however, the discussion does not necessarily have to lead to a new law. "While there is a law regulating the 'Biosiegel,' I don't think we have to have one for regional labeling. All we need is a standard commonly agreed on, with neutral institutions to control adherence to this standard."

Chinese consumers are falling out of love with fakes

WHEN Da Vinci, a retailer of expensive imported furniture, opened its new showroom in Shanghai recently, it spared no expense. The gallery, over 10,000 square metres spread over four stories, was filled with extravagant pieces from brands such as Armani Casa and Versace Home. The theme of the event was zhen de jia bu liao (roughly: “what is genuine cannot be counterfeited.”).

Yet Da Vinci is embroiled in a scandal. CCTV, an official media outlet, alleged that some of its imported kit may actually have been made locally, shipped overseas or to a bonded warehouse, then brought back into the country to earn an undeserved “imported” seal. The firm hired a public-relations agency to put a more positive spin on the story. Da Vinci claims this involved paying $150,000 through a broker to a journalist who, it alleges, threatened to run more damaging stories if not paid off.

All parties involved deny wrongdoing. An initial official ruling seemed to clear Da Vinci, but in December the Shanghai authorities slapped fines on the firm for alleged misdeeds including improper labelling. Da Vinci now says it will take the authorities to court to clear its name and has filed a police complaint against the broker and journalist over what it says was an extortion plot.

Whatever the truth behind this murky affair, it has revealed something about how the attitudes of Chinese consumers are changing. Counterfeiters are no longer popular. Not long ago, Chinese shoppers applauded the fakers for saving them money. Now they scorn them. If it’s a fake, the well-heeled sneer, you can’t flaunt it.

Fakery is not dead, of course. In 2009, roughly 30% of mobile phones in the country were thought to be shanzhai—a popular term for clever fakes. The Business Software Alliance, a trade group, claims that nearly four-fifths of the software sold in China in 2010 was pirated. In December the US Trade Representative issued its annual report on the world’s most “notorious” counterfeit markets. Of the 30-odd markets identified, eight were in China. Some, such as Beijing’s Silk Street market, are well-known. The report also points the finger at Taobao, an online marketplace owned by Alibaba, China’s biggest e-commerce firm. That may be unfair. Taobao has clamped down so hard recently that it is enduring protests by angry vendors.

Still, as China grows richer, life is growing harder for fakers. A recent study of China’s luxury market by Bain, a consultancy, concludes that “demand for counterfeit products is decreasing fast.” McKinsey, another consultancy, found that the proportion of consumers who said they were willing to buy fake jewellery dropped from 31% in 2008 to 12% last year. This is good news for all brands, not just the blingy ones. “Consumers are looking for the real thing, and they are increasingly willing and able to afford it,” say the authors.

The real bling

Cash-strapped youngsters still love counterfeits, says Chen Junsong of the China Europe International Business School. But those over 30, if they have a bit of money, have become extremely brand-conscious. There is a “comparison culture”, he observes. People are ridiculed if spotted with a fake Gucci handbag.

Another reason why fakers are under pressure is that Chinese firms now have intellectual property of their own to protect. Brands such as Lenovo (a computer firm) and Haier (a maker of everything from fridges to air-conditioners) are highly valuable and therefore worth defending. The more Chinese innovators gripe about fakery, the more strictly the government enforces the law. It just announced that it aims to stamp out counterfeit software in government offices by the end of this year.

Even sceptical foreign manufacturers believe China is changing. Douglas Clark, an expert on China’s intellectual-property regime, points to a survey by the EU-China Chamber of Commerce suggesting that counterfeiting, long the first or second gravest concern of its members, is now third, fourth or not even ranked.

…and the bonfire of the bogus

Tougher law enforcement has helped, he says, but so too has the fact that foreign firms have learned how to cope with fakes. Some have set up their own branded retail outlets to control distribution. GlaxoSmithKline, a British drugs giant, has introduced fake-proof “e-coding” of pills.

Chinese consumers are realising that brands are not just about showing off. They can also send useful signals about quality. A Coke probably won’t poison you because it would cause billions of dollars of harm to the Coca-Cola brand if it did.

In December China suffered another scandal involving tainted milk. Shoppers instantly shunned the local firm involved. Nestlé, a Swiss food giant, saw an opportunity. It announced this week that with its local partners it will invest some $400m to boost its dairy operations in China.

Hazard: Product testing demonstrated that these helmets do not comply with CPSC safety standards for impact resistance. Consumers could suffer impact head injuries in a fall.

Description: The recalled items are multi-purpose helmets also sold for use as bicycle helmets. Little Tricky helmets are marketed for children and youth, and feature a large Little Tricky logo on both sides of the helmet. They come in one size and in black, white, pink and green. Triple Eight S/M EPS Liner helmets feature a hard black inner EPS foam liner and come in black, white, bone, blue and army green. Sector 9 S/M EPS Liner helmets feature the same EPS liner and come in gray, white, black, blue and green. Both the Triple Eight and Sector 9 helmets have an interior label indicating the size “S/M” for small/medium and a manufacture date indicated as month/year (ex. APR/2011). Only Triple Eight and Sector 9 size “S/M” EPS Liner helmets are affected.

The ISO 4217 code list is used in banking and business globally. 我跟N公司大戰試圖去說明原因不在我們

Chen: N公司裡面已經有人說我像個律師

me: 哈有意思說不定N 公司指定的廠商和防霉方式有問題

Chen: 他試圖指控工廠的製程有水

但我說一來, 水是N公司唯一認證可用於清潔鞋面的東西

二來, 越南的相對溼度在80%以上很正常, 雨季甚至高達95以上

me: 製鞋製程的機會小這可能是原材料的問題吧

Chen: 當然, 我認為是材料問題

至少不是工廠的問題

材料是由N公司指定的供應商供應的

而N 公司在spec裡面並未清楚標示防黴的需求

me: 這需要檢討supplier的製程和周期內的全製造方式

防黴規格必須補入這要求是commmon sense

Chen: 其實我後來調查, 因為胞子無所不在, 只要環境合適就會長出

所以大部分廠商都是在內盒裡面加一片防黴的貼片

me: 紡織業早對於此有方法應付

Chen: 那個貼片可以釋放出抑制胞子成長的化學物質

me: 我認為治本在皮革處理

Chen: 對, 但是處理過頭會影響材料物性, 比方有個怪味

或是顏色會跑掉

me: 這的確兩難不過可以解決

Chen: 但對N公司而言, 他們很為難

因為他們認為可以殺死黴菌的東西絕對會使某部份人產生過敏反應

衣服會洗過後穿鞋子不會

所以直接處理皮料他們很為難

me: 所以趁機會與N 公司爭議弄好才無後患

Chen: 所以現在他們要去解釋這個差異了

國家質檢總局上週六公告稱﹐對蒙牛乳業（眉山）有限公司生產日期（批號）為2011年10月18日的純牛奶抽查結果顯示﹐黃曲霉毒素M1項目不合格。

黃曲霉素是一種劇毒物和強致癌物質。

國家質檢總局稱﹐中國牛奶中黃曲霉毒素M1的合格標準值為小於等於每千克0.5微克﹐而此次蒙牛牛奶檢驗結果中﹐黃曲霉毒素M1實測值為每千克1.2微克。

內蒙古蒙牛乳業（集團）股份有限公司隨即表示﹐公司立即對該批次全部產品進行了銷毀﹐並向全國消費者道歉。

1983年11月8日(我近30年之後才注意到出版日)，台北的徐氏基金會出版我翻譯的《可靠性入門》Reliability by Arnold Kaufmann (Paperback - 1972)。我的「編譯序」如下：

《可靠性入門》譯序

「可靠性」這門學問，經過數十年來的研究和應用，已成為系統科學的一大主流。然而其文獻和資料太過艱難和浩瀚，所以青年學者每每不得其門而入，只有望「可靠性」而興嘆。鄙人有感於此，特根據昔日留英所購之一本精彩導論，編譯了惡本《可靠性入門》。原書為斐譽世界的法國數學家 M. ARNOLD KAUFMANN 等人為法國的青年所寫，由於深入淺出，精彩無比，所以英國的空中 (開放)大學列為參考書。

本書旨在幫助青年學生 (高中生就可以)和社會人士了解「可靠性」這門日益重要的學問》所以但求能言簡意賅，能幫助讀者很快掌握「可靠性」，並用許多例子來輔助關鍵性概念的說明，希望這種刻意安排，能幫助讀者很快掌握「可靠性」的各重要主題。

玻璃噴飛飄焦味大同：去年已召回 2011年 12月25日民眾使用購買兩年多的大同電子式省電護眼檯燈時，燈泡塑膠殼與燈管居然因爆炸而震開，雖然未傷及眼睛與臉部，但他已嚇得不敢再用。對此，大同綜合訊電公司表示，省電燈泡型號LS-3U21WD，序號為0904、0905、0906的產品確實有瑕疵，去年7月已公告召回，消費者可到全國大同3C門市免費更換。消基會指出，業者應全數檢驗確保燈泡電容穩定後再上市。投訴組陳先生表示，前年8月在大賣場購買一款大同電子式省電護眼檯燈給70歲母親使用，檯燈附贈型號LS-3U21WD的大同省電燈泡。

母親將檯燈擺放在臥室床頭邊使用，每日使用約3個小時，11月30日晚間使用中燈泡卻突然炸開，細小的燈管玻璃碎片四處散落，當時躺在床邊的母親簡直嚇壞了，不過幸好沒受傷。
可至各門市免費換陳先生說，他檢視炸開的燈泡，發現其塑膠殼、燈管都破裂，整個省電燈泡更充斥著燒焦味，隨即致電大同綜合訊電公司反映，希望大同合理回應說明。

對此，大同綜合訊電公司公關經理蔡智鎧表示，2009年4月15日至6月15日產製型號LS-3U21WD，序號0904、0905、0906的省電燈泡，部分電容有容量不足現象，確實會造成燈泡塑膠殼與燈管故障，去年7月有公告召回，目前消費者仍可到全國大同3C門市免費換同等級的燈泡，也會與陳先生協調，處理善後事宜。
「確保安全再上市」消基會監察人暨台灣科技大學電機系副教授蕭弘清指出，省電燈泡壽命已盡時，只會熄滅不再亮，發生燈泡塑膠殼與燈管爆炸震開的異常現象，顯見該款省電燈泡電容有瑕疵。

他指，民眾使用檯燈時，通常與臉部距離很近，若發生爆炸碎裂，很容易傷到眼睛，業者製產販售省電燈泡前，應全數檢驗確保燈泡電容穩定後再上市。

新幹線停電、因部品差「２mili」

新幹線の停電、「２ミリ」が原因　ＪＲ東海

　東海道新幹線の三島―静岡間上り線で１７日深夜に起き、新幹線が最大約５時間遅れた停電について、ＪＲ東海は２０日、送電設備の工事で、規格と２ミリ異なる部品を使ったことが原因と発表した。

　同社によると、新幹線の送電線はボルトとナット、ボルトに押し入れてナットを支えるピンで固定する。ピンとボルトの穴は直径４ミリに統一しているが、１９９０年の工事で業者が同６ミリの部品を使った。９９年にピンのみ同４ミリに交換し、緩んでショートしたという。

　この停電では、計約１１００人が乗った新幹線２本が立ち往生した。

China has promised another wave of tough measures against counterfeiters as it defends itself against constant criticism of its record in the protection of intellectual property rights.

中國已承諾採取又一波打擊假冒商品的嚴厲措施。面對外界對其知識產權保護記錄的不斷批評，中國正尋求維護自己的形象。

The country will launch a crackdown early next year on goods including fake medicines and cigarettes, pirated software and CDs, said Jiang Zengwei, deputy minister of commerce.

中國商務部副部長姜增偉表示，中國將在明年初發起“打假保知”行動，打擊對象包括假藥、假煙、盜版軟件和CD。

The initiative was announced just hours after the office of the US Trade Representative blasted China again over a string of market access hurdles including intellectual property rights protection.

中國宣布這一舉措僅數小時前，美國貿易代表辦公室就一系列市場准入障礙（包括知識產權保護）再度抨擊中國。

It is the first policy announcement since Beijing set up a permanent mechanism to deal with IPR issues last month, including a “leadership group”, which is in effect a collection of politicians operating outside the cabinet with responsibility for dealing with infringements.

The Business Software Alliance, the world's leading software industry body, warned two months ago that revenues lost to software piracy in China were likely to jump because Beijing's high-profile anti-piracy campaigns had failed to deliver.

彰化、台南地檢署昨天在新北、台中、彰化、嘉義等縣市查獲龐大過期食品，查出荷亞商行涉嫌收購過期食品，重印製造日期，變造保存期限，低價賣到夜市及傳統市場。義美、統一、味丹、維力等知名品牌食品都遭變造，商行負責人陳政利每月至少獲利上千萬元，檢方偵訊後聲押獲准。

檢警清查，過期食品五花八門，從泡麵、咖啡、奶粉、餅乾到雞粉、紙尿布、洗髮乳等一百多種，共查獲已遭變造日期的食品二百七十九箱及一千三百五十瓶，「等待變造」的過期品則超過五千箱。

檢方認為這只是冰山一角，許多在傳統市場、夜市販售的廉價食品可能都有問題；另價格便宜的吃到飽飲食店、附贈飲料的便當店，也可能買到過期食品，建議衛生單位加強稽查。

檢方指出，包括開喜烏龍茶、孔雀餅干、味味Ａ泡麵、張君雅小妹妹泡麵、古坑咖啡、ＯＡＫ奶粉等，國內知名食品廠統一、味丹、開喜、旺旺都遭波及，不少過期食品已被民眾吃下肚。

警方接獲檢舉，位在彰化縣秀水鄉荷亞商店涉嫌大量購買過期食品，重新包裝竄改有效日期後，以原價一至五折當成新品賣到傳統市場、夜市、雜貨店，因價格低，不少民眾搶購。

檢警查出，荷亞商行七年前開始販售黑心貨，起先以去光水等藥劑塗掉食品或商品製造日期，再用印蓋上新日期；兩年前購置日期打印機，直接打上日期。

檢警昨天查獲荷亞商行涉嫌收購大批過期食品，在新北市囤貨倉庫查獲變造工具打印機、鉛字、橡皮章、空白貼紙等。

彰化、台南地檢署指揮警方昨天同步搜索荷亞總公司，及新北市、台中市、彰化縣、嘉義市四處倉庫，發現烏龍茶八月就過期，即溶杯裝咖啡更在三月就過期，甚至已停售一年多的飲料，荷亞仍繼續販售。

新北市倉庫裡堆滿近千箱從東南亞進口的餅乾、糖果、飲料，及知名國產品牌義美、旺旺、維力、台鳳、森永等產品，包括義美荷仁豆、美人新肌飲、乖乖玉米棒、浪味先等三百八十七箱過期品。台中工廠發現麗仕精華護髮、孔雀餅乾、味味一品、李錦記鮮味雞粉、多芬深層修護髮膜等過期品。

荷亞商行負責人陳政利否認變造有效日期，辯稱買來的都是即將到期的食品，不肯透露貨物來源，只說是找人到處收購。

檢方認為荷亞能夠大量鋪貨，懷疑有連鎖賣場或商店固定提供貨源，偵訊後將陳政利聲押獲准，陳妻徐鳳珠和兩名員工分別以十萬元和三萬元交保。

The machine has a touch-screen console, an adjustable fan and an orange-colored, oversized water bottle holder. A medallion with the Schwinn logo is on the bottom right side of the base of the machine.

TOKYO — Japan's Honda Motor is recalling more than 300,000 cars worldwide, including models of the popular Accord and Civic, because of a defect in the driver's airbag, the company said Friday.

In the worst case scenario, the gas container which inflates the airbag during an accident could break and send fragments of the safety device scattering, the company said in a statement.

The automaker will recall 304,035 units of 10 models manufactured in 2001 and 2002, it said.

Some 300,000 of the cars under recall were sold in the United States and Canada, Honda said, while the remainder were sold in Japan and other regions.

The company said it will replace the defective inflator with a new one in the recalled cars.

Honda said the latest recall was an extension of previous ones issued between 2008 and 2010, covering about 950,000 vehicles.

Japan's car giants have carried out millions of recalls in the past year and a half as they have become more proactive about dealing with faults.

Fellow Japanese automaker Toyota, previously lauded for its safety standards, became mired in crisis when it recalled nearly nine million vehicles between late 2009 and February 2010 due to brake and accelerator defects.

Australian air safety officials have launched an investigation after an airline passenger's Apple iPhone apparently caught fire during a flight.

Local airline Regional Express said the phone began glowing red and emitting dense smoke after the plane landed in Sydney on Friday.

Flight attendants had to douse the phone, which has since been handed to investigators.

Officials said they would be consulting Apple as part of their inquiry.

"We have no previous records of iPhones undergoing spontaneous self-ignition," a spokesman for the Australian Transport and Safety Bureau (ATSB) told AFP news agency.

"We do have the phone, it's in our custody, and we will be undertaking a technical examination of it."

Apple said it was looking forward to working with the investigators.

（台灣‧台北27日訊）台灣消費組織抽檢驚人發現，當地市面上逾70%茶包、滷包等，竟是塑膠類而非一般認知的紙類，遇高水溫將溶出塑化劑等有毒物質，長期攝取更可能威脅健康。

台灣消費者保護協會最新抽查發現，市面70%茶包是用塑料製裝載茶葉袋，內含PET塑膠物料（即製作塑膠瓶的材料），若泡在超過60℃的熱水中，會釋出塑化劑及鉛、鎘等重金屬，傷害心臟、肝臟。

而標籤紙浸水後，油墨也可釋出有害化學物質高錳酸鉀，雖然含量無超標，但長期在身體積聚，仍會傷害肝臟。

消保會組長吳政學表示，市民對茶包滷包包裝袋的認知都是紙類，一般都會長時間泡在高溫裡。他認為，塑膠類茶包若泡在100℃的熱水逾30分鐘，民眾喝下恐影響健康。

林口長庚醫院毒物科主任林傑樑說，茶包等材質都不應為塑膠，尤其是PET，長期或大量使用恐導致心臟及肝臟受損。

British Airways Flight 5390 was a British Airways flight between Birmingham Airport in England and Málaga, Spain. On 10 June 1990 an improperly installed panel of the windscreen failed, blowing the plane's captain, Tim Lancaster, halfway out of the aircraft, with his body firmly pressed against the window frame. The first officer managed to perform an emergency landing in Southampton with no loss of life. "British Airways Flight 5390" is now used as codeshare route between Des Moines, IA USA and Chicago-O'Hare, IL USA operated by American Airlines.

The aircraft, captained by Tim Lancaster and co-piloted by Alastair Atchison, was BAC One-Eleven Series 528FL G-BJRT. It took off at 07:20 local time, with 81 passengers, four cabin crew and two flight crew. Co-pilot Atchison handled a routine take-off, and relinquished control to Lancaster as the plane established itself in its climb. Both pilots subsequently released their shoulder harnesses, while Lancaster loosened his lap belt as well.

At 07:33, the cabin crew had begun to prepare for meal service. The plane had climbed to 17300 feet over Didcot, Oxfordshire. Suddenly, there was a loud bang, and the fuselage quickly filled with condensation. The left windscreen, on the captain's side of the cockpit, had suffered a catastrophic failure. Lancaster was jerked out of his seat by the rushing air and forced head first out of the cockpit, his knees snagging onto the flight controls. This left him with his whole upper torso out of the aircraft, and only his legs inside. The door to the flight deck was blown out onto the radio and navigation console, blocking the throttle control which caused the plane to continue gaining speed as they descended, while papers and other debris in the passenger cabin began blowing towards the cockpit. On the flight deck at the time, flight attendant Nigel Ogden quickly latched his hands onto the captain's belt. Susan Price and another flight attendant began to reassure passengers, secure loose objects, and take up emergency positions. Meanwhile, Lancaster was being battered and frozen in the 500 mph slipstream, and was losing consciousness due to the thin air.

Atchison began an emergency descent, re-engaged the temporarily disabled autopilot, and broadcast a distress call. Due to rushing air on the flight deck, he was unable to hear the response from air traffic control. The difficulty in establishing two-way communication led to a delay in British Airways being informed of the emergency and consequently delayed the implementation of the British Airways Emergency Procedure Information Centre plan.

Ogden, still latched onto Lancaster, had begun to suffer from frostbite, bruising and exhaustion. He was relieved by the remaining two flight attendants. By this time Lancaster had already shifted an additional six to eight inches out the window. From the flight deck, the flight and cabin crew were able to view his head and torso through the left direct vision window. Lancaster's face was continuously hitting the direct vision window; when cabin crew saw this and noticed that Lancaster's eyes were opened but not blinking despite the force against the window, they assumed that Lancaster was dead. The reason the cabin crew did not release Lancaster's body despite the assumption of his death was because they knew that releasing his body may cause it to fly into the left engine and cause a engine fire or failure which would cause further problems for Atchison in an already highly stressful environment.

Atchison eventually received clearance from air traffic control to land at Southampton, while the flight attendants managed to free and hold on to Lancaster's ankles for the remainder of the flight. By 07:55 the aircraft had landed safely on Runway 02 at Southampton. Passengers immediately disembarked from the front and rear stairs, and emergency crews retrieved Lancaster.

Lancaster was taken to Southampton General Hospital, where he was found to be suffering from frostbite, bruising and shock, and fractures to his right arm, left thumb and right wrist. Flight attendant Nigel Ogden suffered a dislocated shoulder, frostbitten face and some frostbite damage to his left eye. He retired in 2001 due to stress, and went on to work the night watch at a Salvation Army hospital. Everyone else left the aircraft unhurt.^[1]

Less than six months after the accident Lancaster was working again. He later left British Airways when he reached the company's mandatory retirement age. As of 2005, Lancaster was flying for easyJet.^[1]

[edit] Investigation

Accident investigators found that a replacement windscreen had been installed 27 hours before the flight, and that the procedure had been approved by the Shift Maintenance Manager. However, 84 of the 90 windscreen retention bolts were 0.026 inches (0.66 mm) too small in diameter, while the remaining six were 0.1 inches (2.5 mm) too short. The investigation revealed that the previous windscreen had been fitted with incorrect bolts, which had been replaced on a "like for like" basis by the Shift Maintenance Manager without reference to the maintenance documentation.^[2] The air pressure difference between the cabin and the outside during the flight proved to be too much, leading to the failure of the windscreen. The incident also brought to attention a design flaw in the aircraft of the windscreen being secured from the outside of the aircraft, putting a greater pressure on the bolts than if they were secured from the inside.

Investigators blamed the British Airways Birmingham Airport Shift Maintenance Manager for installing the incorrect bolts during the windscreen replacement and for failing to follow official British Airways policies. They also found fault with British Airways' policies, which should have required testing or verification by another individual for this critical task. Finally, investigators blamed the local Birmingham Airport management for not directly monitoring the Shift Maintenance Manager's working practices. The AAIB chief investigator was Stuart Culling.

Investigators made eight safety recommendations in the final accident report:

Review their quality assurance system and encourage engineers to provide feedback.
Review the need to introduce job descriptions and terms of reference for engineering grades Shift Maintenance Manager and above.
Review their product sample procedure to achieve independent assessment of standards and to conduct an in-depth audit into the work practices at Birmingham Airport.

Examine the continued viability of self-certification with regards to safety critical tasks on aircraft.
Review the purpose and scope of the FOI 7 Supervisory Visit.
Consider the need for the periodic training and testing of engineers.
Recognise the need for the use of corrective glasses, if prescribed, in association with aircraft engineering tasks.
Ensure that, prior to the issue of an air traffic control rating, a candidate shall undergo an approved course including training in both the theoretical and practical handling of emergency situations.

Description: This recall involves a wardrobe door with full-length glass mirror with model number 101-604-51, supplier number 12650 and production weeks 1039 through 1048 (YYWW). The product name "PAX AURLAND" and "IKEA" and production week are printed on the rear of the mirror door on the lower edge. The mirror door is made of foil-coated particleboard and the mirror of glass. The mirror is secured with metal fastenings at top and bottom. The mirror door measures about 191/2 inches wide, 93 inches high and 7/8 inches thick.

Sold exclusively at: IKEA stores nationwide from October 2010 through September 2011 for about $60.

U.S. officials say a problem that has long plagued luxury handbag makers such as Gucci and Louis Vuitton is now afflicting the Pentagon’s high-end weapons systems: cheap Chinese counterfeits.

A months-long congressional probe found at least 1,800 cases of counterfeit electronics in U.S. weapons, with the total number of suspect parts exceeding 1 million.

The results of the investigation, conducted by the Senate Armed Services Committee, are to be presented at a hearing Tuesday, where senators plan to grill defense contractors about lapses in monitoring their parts supply chain.

“We cannot allow our national security to depend on electronic scrap salvaged from trash heaps by Chinese counterfeiters,” said committee Chairman Carl Levin (D-Mich.). He called the report’s findings — based on records from 10 defense contractors and their testers — “just the tip of the iceberg.”

In more than 70 percent of the cases in which investigators traced parts back to their source, the trail led to China. And nearly 20 percent of the remainder were traced to Britain and Canada — resale points for counterfeit Chinese parts, Senate staffers said.

Chinese Embassy spokesman Wang Baodong said that his government’s position “on striking down on fake products is clear and consistent.” He said China’s policy “is aimed to ensure that commodities of safety and quality are provided to the consumers.”

In the past, the embassy also has said that the issue of investigating counterfeit parts was a matter of “China’s judicial sovereignty.”

Fake electronic parts often are produced in China by burning raw material off old circuit boards, washing the components in sometimes-polluted rivers and drying them on city sidewalks, Senate investigators said. They said the resulting parts are unreliable over the long term, even if they pass initial factory testing by manufacturers.

“We can’t tolerate the risk of a ballistic missile interceptor failing to hit its target, a helicopter pilot unable to fire his missiles or any other mission failure because of a counterfeit part,” said Sen. John McCain (Ariz.), the ranking Republican on the Armed Services Committee.

McCain and Levin said they intend to use the 2012 Defense Authorization Act to modify acquisition provisions so that the onus is on contractors to pay for replacing such parts, with the hope that they will adopt stricter policies with their suppliers.

The lawmakers also threatened to seek the inspection of shipments of all Chinese electronic parts — for military and commercial use — at U.S. ports if China does not take steps to curb the flow of counterfeit goods. The costs of those inspections, they said, would be borne by the shippers.

In a briefing Monday, Levin angrily accused Beijing of allowing a “brazenly open market” for counterfeit parts in Shenzhen, Guangdong province. He noted that committee staff members were stopped in Hong Kong and refused visas into China by officials who warned that their investigation was “sensitive and could be damaging to U.S.-China relations.”

Levin said that China’s authoritarian rulers could stop the counterfeiting “if they want to stop it.”

Counterfeit electronics parts are a well-known scourge of the military supply chain, and China a well-known source. A 2010 study by the Commerce Department said that China was nearly five times more likely than any other country to be identified as the source of suspected counterfeit goods by manufacturers, distributors and others involved in the supply of weaponry to the Defense Department.

The recycled materials used in those parts are often sold overseas through a complex web of suppliers, contractors and subcontractors. And the use of counterfeit parts, Senate staffers said, has at times resulted in millions of dollars in waste, with U.S. taxpayers footing the bill when contractors discover the need for replacements.

北京恐怕是全球空氣污染最嚴重的大都市之一。不過在喘氣的問題上，不是讓領導先喘，而是讓領導"優先喘"。在中南海辦公的領導人們享受著經過過濾的干淨空氣。而對普通民眾公佈的空氣質量監控數據已經得到"美化"。

據德新社報導，多年來很多中國人都感到驚訝，北京的空氣質量這麼差，為什麼中央的領導們卻袖手旁觀呢？他們不是要呼吸同樣臟的空氣嗎？不過直到現在這些老百姓才了解到，這些"勞心者"們活動的區域空氣已經得到有效地淨化了。在黨的高官幹部們辦公和生活的場所安裝了遠大集團生產的空氣淨化機。

德新社的報導稱，遠大在公司網頁上曾登載了一篇"遠大空氣淨化機成功案例-中南海篇"。這篇廣告軟文中稱，遠大空氣淨化機走進"中國政治生活的心臟"；稱"遠大淨化機為國家領導人創造了健康，清新的辦公環境，是百姓之福"。

但是對於北京普通市民來說，面對嚴重的空氣污染，他們卻束手無策。一位網民在網上寫道："只有黨和國家領導人以及他們的家人也吃被污染的食品，也喝被污染的水，也喘被污染的氣的時候，我們才有希望真正改善空氣質量。"

美國駐華使館監測北京空氣質量發出嚴重污染警告時，北京環保局卻輕描淡寫成"輕度污染"，引起民眾的強烈不滿。

按照世界衛生組織所建議的，美國駐華使館對空氣中的細顆粒物(PM2.5)，也就是大氣中直徑小於或等於2.5微米的顆粒物進行監測。而北京市環保局監測的是PM10，也就是粒徑小於等於10微米的顆粒物。北京市環保局副局長杜少中上週在接受北京一家媒體採訪時說："我不清楚他們（美國駐華使館）的監測手段和方法，如何保證準確性。但我覺得他們公佈的方式更像是炒作，不是很嚴謹的做研究的態度。"

根據美國駐華大使館在推特上定時發布的使館空氣監測站數據，北京朝陽區空氣質量目前屬“危險級別”。北京市環保局發言人則指責美使館測量數據不準確，並有“炒作”之嫌。北京百姓不知到底該信誰的數據，索性呼籲自發進行測量。

北京連日持續大霧天氣，能見度低。本周初，美國駐華使館大樓上的空氣質量檢測站測量的結果顯示，使館所在的北京朝陽區的空氣污染的細顆粒物(PM2.5)指標已經嚴重超標，北京的空氣屬重度污染，對人體健康威脅程度已經進入"危險級別"。

美國駐華使館空氣監測數據引起廣泛關注,與房地產大亨潘石屹上週末在微博上分享的一張截圖不無關聯。該圖顯示10月30日12時，北京空氣質量指數為425，PM2.5濃度為387，級別為"有毒害"。上述數據的來源是美國使館的監測站。這條微博記錄引來大量轉發和評論。有網友隨即轉發了北京市環保局當天發布的空氣測量數據：空氣污染指數132，屬輕度污染。

美中測量標準不同

根據美國駐華使館提供的信息，位於朝陽區的使館的空氣監測站設立於2008年，監測標準是按照美國的標準針對空氣中細顆粒物PM2.5，是指大氣中直徑小於或等於2.5微米的顆粒物，也稱為可入肺顆粒物。它的直徑還不到人的頭髮絲粗細的千分之五。與較大顆粒物相比，PM2.5粒徑小，富含大量的有毒有害物質且在大氣中停留時間長、輸送距離遠，因而對人體健康和大氣環境質量的影響更大。

對於美國駐華使館的測量數據，北京市環保局官員杜少中認為，美使館數據測量的只是一個點，不能代表一個區域。杜少中11月2日在接受媒體採訪時說，"北京市環保局共有27個監測點，評價一個大城市的空氣質量，需要一個系統，不能由一個點作為評判。"11月4日法新社報導中援引杜少中說，美方監測的數值不精確，甚至有"炒作"之嫌。杜少中還補充說，美國大使館發布的是濃度，環保局發布的是指數，兩者間需要換算。此外，北京市環保局監測的標準也有別於美國使館，是PM10，是指直徑10微米以下的顆粒物，又稱為可吸入顆粒物或飄塵。

百姓"心裡有桿秤"

一邊是美國駐華使館的獨立監測，一邊是北京官方的測量結果。有出入的數據讓普通百姓有些摸不著頭腦。雙方的數據該相信誰呢？北京退休女工馮女士說："百姓誰說得好啊？相信人家美國？人家可能比較客觀。中國總愛掖著藏著。"北京在職老幹部林先生說："我信中國。中國是為百姓服務的國家。不過這幾天天氣是不好，氣壓低，大霧。"北京某公司高級白領嚴小姐說數據的出入可能是由於標準不同："因為美國使館只測一個點，不全面，而且兩邊的標準也不同。但是這幾天天氣特別不好，環保局說的輕度到中度污染，我倒是不信，不過像美國使館說得那麼嚴重，可能也不至於。"媒體報導說，北京市上週末各醫院接收的呼吸道疾病患者人數增加了13%。北京宣武某醫院的白醫生解釋說："現在這個季節本身就是呼吸道疾病比如哮喘的高發期，倒不一定主要是空氣污染造成。"對於美國使館與北京環保局的測量出入，白醫生認為："可能有誇張的，也有不實報導的，一般我更願意相信二者之間的那個數字。"

美國駐華使館每隔一小時，在大使館網頁和推特頁面同步更新空氣測量結果。截至發稿，美國使館發布的11月4日北京空氣質量是PM2.5為143.0, 空氣質量等級為"不健康"。北京市環保局網頁上公佈的4日北京空氣質量狀況是"良到輕微污染"。

YOU can’t live with them, but you can’t live without them. Users of BlackBerry smartphones often curse the flashing red light that tells them, wherever they are, that yet another e-mail demands their attention. But they curse its absence even more. On October 11th lots of them, in Europe, the Middle East, Africa, India and South America, had to twiddle their thumbs rather than click away at their smartphones. For many, service had been interrupted for the second day running. On October 12th problems were reported in North America, too. Plenty tweeted their rage at Research in Motion (RIM), the Canadian company that makes BlackBerrys—presumably from other devices.

For RIM, the interruptions could scarcely have come at a worse time. The company has had a bad year. In its most recent quarter (ending in August) its revenues were 10% lower than a year before, at $4.2 billion. Its profit, at $329m, was down by more than half. According to Gartner, a research and consulting firm, in the second (calendar) quarter RIM’s share of the smartphone market declined to 12%, from 19% a year earlier.

BlackBerrys, it is true, still have lots of enthusiastic followers. Commuters and corporate road warriors needing to keep in touch with colleagues and clients swear by them, as well as into them and at them. A QWERTY keyboard of buttons is kind to clumsy middle-aged thumbs (though touch screens are also on offer). Information-technology departments, particularly in companies that put a special premium on security, love them too: life is fairly simple with a PC on every desk and a BlackBerry, connected to the corporate e-mail system, in every hand. And many youngsters, especially in Britain, have taken eagerly to BlackBerry Messenger (BBM), a free instant-messaging service. Less happily, politicians called for BBM to be shut temporarily during this summer’s English riots, because some were using it to organise disturbances.

But the smartphone business is changing—away from RIM. Not only a growing number of road warriors, but also their bosses, want to use their own smartphones rather than company-issue BlackBerrys, and are increasingly allowed to do so. Apple’s iPhone and the many Android-based competitors boast many more apps, and are simply trendier. Ageing men in suits can be fashion slaves, too.

It has not helped that RIM was slow to get new products to market, although phones with a new operating system are now appearing. The company is not only smarting, so to speak, in its core business. It has also made a lacklustre entry into the tablet market, shipping 500,000 its PlayBooks in its first quarter but only 200,000 in its second. Gartner describes its tablet operating system, QNX, as “a promising platform”, and thinks RIM may sell 3m units this year. But that compares with nearly 47m iPads, or 73% of the market by volume.

Investors as well as customers are getting impatient. RIM’s share price has fallen by more than 60% since February. Whatever wonders its co-chairmen and chief executives, Jim Balsillie and Mike Lazaridis, have worked in the past, two leaders now look as bad as none. Jaguar Financial, a Canadian investment bank, smells complacency. It wants a change at the top and in direction, perhaps even to break RIM into three: a network company, a device-maker and a holder of patents. It claims the support of the owners of 8% of RIM’s shares.

Bosses and shareholders at Nokia, Hewlett-Packard, Yahoo! and elsewhere have already found that change in the tech world can be disarmingly sudden. Now it is RIM’s turn to suffer a dramatic loss of power. In more ways than one.

為什麼民主國家的立法品質極為重要？因為民主國家需要法治配套，而非人治。人治國家（一個人獨裁統治或寡頭少數統治或一個黨的一群人統治）領導人及其政黨高於法律之上，立法品質就不重要了。立法越含糊越好，便於統治者做各種有利於統治的法律解釋，例如以前的《戒嚴法》，一句定義不明的「挑撥政府與民間的感情」就可以抓人、封刊物。

民主國法條應精確

民主國家立法很講究定義的清晰明白，因為民眾感覺權益受損必須和政府打官司，若定義不明易遭政府侵權，因此立法品質就是人民權益的保障。民主和獨裁的一個重大差別在於政府的可預測性。獨裁國家一切事物都「說不準」，今天這樣，明天又改那樣，誰也不知道現行政策和規定何時又改動，很難預測。去中國做生意的人一定嘗過這種類似的苦頭。

民主法治國家盡量讓政策具有可預測性；而建立穩定的法律系統，精確的法條敘述，是其要件。換句話說，立法品質是建立政府可預測性的基礎。不可預測的政府是危險又恐怖的。
台灣立法品質低落，不但條文的撰寫不夠高明，與別的法律相矛盾也無法察覺。品質超爛的立法有四大壞處：民眾不知如何遵守。敘述太過抽象或晦暗以致窒礙難行。法官也眾說紛紜，各有各的見解，以致同一法條在不同法官引用時會出現多重標準，產生多種不同的判決、難以服眾。人民難以預測後果。

竟誤植內容漏條文

立法院今年6月修訂《國民年金法》，二讀時才發現漏列部分修正條文，將使地方政府若拖欠國保保費，政院失去強制扣抵補助款的依據。議事人員說，法案有十幾個版本是家常便飯，一到清倉期，議事人員或行政機關若沒留意就容易出錯。
這不是今年首例。6月14日會期最後一晚，議事人員宣讀尚未協商完成的《內政部組織法》，還完成三讀。朝野黨團發現，緊急提復議處理。
6月19日，《食品衛生管理法》竟把「停業一定期間或歇業」，誤植為「歇業一定期間或停業」，形同只能處罰暫停營業。8月27日，《莫拉克颱風災後重建特別條例》第16條1、2、3項，與第19條1、2、9項內容重複。
立委受到各種利益團體的壓迫，導致有些條文文字曖昧不明，不知所云，以留給利益團體迴旋的空間。如此立法品質是「頭頂生瘡，腳底流膿－爛透了！」

　「あきらめないで！」のCMでも有名な悠香の「茶のしずく石鹸」だが、同製品を使用した女性がパンなどの小麦製品を食べた後に、アレルギーによるショック症状を起こす事例が数多く起こっている。2008年あたりからこの様なケースが医療関係者から報告されていて、昨年10月には厚生労働省が悠香製品と特定せずに注意喚起を行う事態に発展した。それから半年ほど経った今年5月にメーカーが自主回収を開始。7月頃からは、遂に各地で弁護団が発足している。

　弁護団の説明会では被害女性から「アナフィラキシーによるショック症状で2度倒れたが原因が分からず病院を転々とした」「2009年から使い始め妊娠6カ月の時にアナフィラキシーを発症し、何度か病院に搬送された。なすすべなく自宅静養を続けている」などと切々とした訴えが聞かれた。よもや石鹸が原因とは自覚のないままに使用を続け、その結果、原因が分からずに突然襲われたアナフィラキシーによって退職など社会的制約を余議なくされた人も少なくない。また被害女性の多くは原因が特定された今でも小麦製品を摂取することができないために、食生活の手間暇や経済的負担がかかっているという。

　アレルギー問題に詳しい国立病院機構相模原病院アレルギー科の福富友馬医師によると、09年頃から「ちょっと変わった小麦アレルギーが増えてきた」との所感があり、通常の小麦アレルギーであれば主な症状は蕁麻疹だが、今回のケースでは目の痒みが特徴であったという。ただそれが、同製品に起因するものとはすぐには分からなかったのである。

　これらアレルギーを起こす物質として今回問題となっているのは、「茶のしずく石鹸」旧製品に配合されていた"加水分解コムギ"という成分。天然の食物タンパク質を工業的に加水分解したものであり、化粧品やヘアケア製品に保湿成分として含まれているものだ。しかしなぜこれがアレルギー症状を引き起こしたかについては諸説あり、未だ明確にはされていない。

　前出の福富医師によると、成人は肌や粘膜を通して食物アレルギーに至ることがとても多く、花粉やヨモギスパイスが食物アレルギーを引き起こしたケースも稀ではない。「茶のしずく石鹸」は洗顔石鹸であることから、顔を洗った際に目や鼻孔から問題の成分が吸収されアレルギーを引き起こしたのではないかと思われる。最初は肌の赤みや痒み程度だったものが、使い続けるうちにアレルギー症状が悪化する事例も多い。症状は深刻で、パスタやパン、ラーメンなどの小麦製品を食した後に運動（5分程度の歩行を含む）をすると、目の痒みや全身の発赤などが現れ、ひどいケースでは血圧低下、意識喪失という症状に至っている。

　東京弁護側が設けた「被害110番」には8月中の2日間で600件以上の相談があり、潜在的な被害はさらに大きいと見られる。同製品は 2005～06年に発売を開始し通信販売をメインに昨年8月までで少なくとも4,000万個を販売。大々的なコマーシャル戦略を展開し幅広い年齢層の女性に支持を得ていた。

　通常、化粧品による被害は即時的に分かるものがほとんどで、その製品を使うのを止めてしまえば被害はなくなる。今回のように重篤な事態に陥ることは極めて稀なのだが、なぜこれほどまでに被害が拡大したのだろうか。

　弁護団の神山美智子弁護士は「被害はもちろん意図的なものでないとしても、メーカー側の対応にやはり問題があった。厚生労働省の通達から半年以上も積極的な対応をせず、被害者に十分な説明もなかった」と話す。会社側は被害者が医療機関で診断書を作成してもらっても、同社製品による小麦アレルギーだとは認めようとしなかったという。そしてダイレクトメールや手紙、ハガキによる注意喚起はされたものの、まったく情報が浸透していないという現状がある。

　実際、石鹸と小麦アレルギーの因果関係は医療関係者でも困惑するほど分かりづらいものであった。だからこそテレビコマーシャルなど、これまで販促で使ってきたメディアを通して被害を報告する義務があったのだ。被害を最小限に留めることができていれば、集団訴訟という最悪の事態を防ぐことができたからだ。

　一連の騒動は急成長企業に潜む、売ることだけに力点が置かれた体制の脆弱性を露呈した。もちろんこれらは悠香だけの問題では済まされない、業界自体の信頼に関わってくる問題になる。なぜ工業会や学会など業界内にある連携が自浄作用として機能しなかったか、この問いを掘り下げていく必要があるだろう。そして消費者としては化粧品がアレルギーの原因になりうることや、皮膚や粘膜を通して吸収されるさまざまな物質が体内に影響を与えるという可能性を忘れてはならないだろう。

Management: The Lasting Legacy of the Modern Quality Giants

As Quality Magazine celebrates its 50th anniversary, we take a look at some of the quality gurus who have made a significant impact on the world through their contributions to improving not only businesses, but all organizations.

Certainly there have been many quality gurus who have left their mark on industry and society as a whole. This article focuses on six people, three of whom I am proud to say that I have known professionally and personally, who had a profound impact on the quality movement. Subconsciously, I may have placed these luminaries in priority order so this list maybe one-sided and can be argued from various perspectives. As the author, I have taken some liberties but many experts will likely concur with those appearing here but the order may change, depending on individual experiences.

Walter A. Shewhart (1891-1967)

Walter Andrew Shewhart was a giant among giants in the quality movement during the first half of the 20th century. While working for Western Electric located in Hawthorne, IL, his groundbreaking work focused on reduction of variation and charts. His work altered the course of industrial history, led a quality revolution and launched the quality profession.

Shewhart defined the problem of process variability in terms of assignable and chance causes. On May 16, 1924, Shewhart prepared a memorandum of less than one page in length and forwarded to his manager, George Edwards, who later became the first president of American Society for Quality Control. This memorandum set forth the essential principles of controlling variation through the application of control charts.

Shewhart’s principle was that bringing a process into a state of statistical control would allow the distinction between assignable and chance cause variations. By keeping the process in control, it would be possible to predict future output and to economically manage processes. This was the birth of the modern scientific study of process control. Shewhart has often been referred to as the father of statistical process control.

In 1931, he published his watershed work, “Economist Control of Quality of Manufactured Product.” It challenged the inspection-based approach to quality and introduced the modern era of quality management. Up until this time, statistical process control was largely a Bell Telephone quality tool. Shewhart’s book, however, popularized statistical control and its use then spread throughout industry.

From the 1930s forward, Shewhart’s interests expanded from industrial quality to wider concerns in science and statistical inference. Shewhart was at the core of a group of people who were destined to become famous in their time. This group included Harold Dodge and Harry Romig, known for their work on product sampling plans. Joseph M. Juran worked at the Hawthorne Plant and Bell Laboratories for several years and worked closely with Shewhart.

W. Edwards Deming worked as an intern at the Hawthorne Plant where he became interested in Shewhart’s work. Shewhart and Deming had a long relationship of collaboration. Deming continued to champion Shewhart’s ideas, methodologies and theories throughout his career. While working with Japan, Deming further developed some of Shewhart’s methodological proposals of scientific inference, which had been named the Shewhart Cycle and was represented by the plan-do-check-act elements.

During the 1990s, Shewhart’s work was rediscovered by a new generation of industrial engineers and managers, and this time these concepts were repackaged and incorporated into the Six Sigma approach.

Shewhart believed that statistical theory should serve the needs of industry and society as a whole. He challenged the norms of his day and showed manufacturers a better way that revolutionized industry. Shewhart has often been referred to as the father of statistical quality control because he brought together the disciplines of statistics, engineering and economics.

Upon his death in 1967, there were a multitude of commentaries from many who were themselves important figures in the development of the quality field. An excerpt from a speech by the chairman of the committee that awarded the first ASQ Shewhart Medal captured Shewhart’s character in the follow words:

“Shewhart’s legacy lives in mementos of him—a simple bowl and some numbered chips, a bronze medal, some books and writings. It lives in the succession of other prominent individuals he influenced, and it lives in the society of professionals who carry on the work he started.”

Joseph M. Juran (1904-2008)

Joseph Moses Juran had a profound impact on not only the lives of countless individuals, but on nations of the world as he spread the gospel of total quality. No other person was able to capture the essence of quality as a total effort. Juran has been called the father of quality and referred to as the greatest quality giant of the 20th century. Perhaps, more importantly, he is recognized as the person who influenced the addition of the human dimension to quality, broadening it from its statistical origin to the more comprehensive total quality management.

Juran pushed for the education and training of managers. His thought was that human relations problems were the ones to isolate and resolve. Resistance to change was more cultural and the root cause of quality problems.

My personal exposure to Juran began with one of his articles more than 35 years ago. As a young quality engineer, I was part of team charged with bringing to my company unique quality approaches. Juran consented to visit with our senior management and share his thoughts. I had the privilege of meeting him during one of his visits, and after more than 30 years, our company is still applying some of his recommendations.

I was able to meet Juran on other occasions, but during our first meeting I was inspired to sustain a career in quality and become involved in American Society for Quality. Through a journey of studying and learning about quality from a variety of people, I became a Juranite—a disciple of his theories and practioner of his principles on quality. He did not just teach statistics. In fact, he did not think of himself as a statistician, but more dedicated to the total quality approach. Through Juran’s influence it became evident that the human side of quality was just as important as the technical side.

Juran’s life is like a Horatio Alger story. His humble start in life gave no indication to the success and notoriety he would achieve during his life.

In 1924 he accepted a position in the inspection group at Western Electric, a division of AT&T, in Hawthorne, IL. He rose to inspection division chief in just five years. During this time he wrote the first known text on statistical quality control—and the ancestor of today’s widely used “Western Electric Statistical Quality Control Handbook.”

Little did he know at the time the decision to work at Western Electric would set him off on a more than 75-year career in quality. He would work with a virtual “who’s who” in quality. People such as George Edwards, Harold Dodge, Harry Romig and Walter Shewhart were instrumental in shaping his ideas and his future.

During World War II, Juran served the Department of Defense (DoD) as assistant administrator of the Lend-Lease Program. After the war he did not go back to Western Electric but went forward to create history and a legacy for generations of quality professionals.

In 1946 Juran, along with several other notables, founded the American Society for Quality Control. He developed what has been called the foremost influential course on quality. His “managing for quality” has been taught to thousands of people in almost every country of the world.

In 1954 he conducted seminars for Japan’s senior and middle managers, explaining the roles they had to play in promoting quality. Juran was invited back many times and his teachings were so inspirational to the Japanese people that a temple was named in his honor. He also was honored with Japan’s highest award that can be given to a non-Japanese, the Order of the Sacred Treasure. It was awarded in recognition of his contribution to “the development of quality control in Japan and the facilitation of U.S. and Japanese friendship.”

Juran’s process of developing ideas was gradual. Top management involvement, the Pareto principle (which could easily have been named the Juran principle), the need for widespread quality training, the definition of quality, the Juran Trilogy, the project-by-project approach to improvement—these are some of the ideas for which he is best known and all emerged gradually.

The first edition of his classic book, the “Quality Control Handbook,” first released in 1951, contained about 100 pages and his fifth edition contained more than 1,900 pages. (The 6th edition was recently released with much acclaim.) His handbook is still widely considered to be the bible on quality and was instrumental in becoming core elements in the body of knowledge for ASQ certified quality engineers.

Juran founded the Juran Institute in 1979 with the aim of increasing awareness of his theories and principles. It was through this institute that the widely acclaimed video series “Juran on Quality Improvement” was produced. He continued to deliver exceptional contributions well into his 90s. He played a part in development of the Malcolm Baldrige National Quality Award, which has led a revolution of organizational improvement.

Like Shewhart, much of Juran’s work was rediscovered by engineers and managers in the 1990s. His principles of improvement coming project-by-project led by trained facilitators and leaders were incorporated into the Six Sigma approach.

Juran was one of those rare individuals whose collective work spans generations to make a sustained impact on society. His groundbreaking work in quality management and leadership was the catalyst that transformed industries. Shortly before his death Juran said, “My job of contributing to the welfare of my fellow man is my great unfinished business.” He is truly missed and it is doubtful there will be anyone who will soon follow in his footsteps. Juran was truly the “architect of quality.”

William Edwards Deming (1900-1993)

W. Edwards Deming was an American statistician, author, lecturer, professor and management consultant. Perhaps he is best known for his work in Japan after World War II when the Department of Defense (DoD) asked his help in the rebuilding of their industries. From 1950 forward, Deming taught management how to improve design, product quality and testing through various methods, including the application of statistical application.

Deming’s great legacy was that he opened the way for quality and statistical thinking in Japan, and later to American companies such as Ford Motor Co. Deming made a significant contribution to Japan’s reputation for innovative high-quality products and its emergence as an economic power. He is regarded as having had more impact upon Japanese manufacturing and business than any other person of non-Japanese heritage.

Deming is considered a folk hero in Japan because of the impact of his work. His influence was so significant that it is considered to have significant influence on Japan’s third wave of industrial revolution. The Japanese government showed their appreciation for his work by honoring Deming with an imperial award, the Order of the Sacred Treasure, and establishing an award in his name, the Deming Prize.

His accomplishments while working with the Japanese resurging industries could hardly have been predicted but his inspiration for excellence was set in place years before. While working on his Ph.D. at Yale in mathematics and physics, he had internships at Bell Telephone Laboratories, a division of AT&T. Deming found great inspiration in the work of Walter Shewhart, the originator of the concepts of statistical control of processes and the related technical tool of the control chart.

Shewhart’s idea of common and chance causes of variation led directly to Deming’s theory of management. He realized that these concepts could be applied not only to manufacturing processes but also to the processes by which organizations are led and managed. This insight was significant as it made possible Deming’s influence on the economics of the industrialized world after his work with the Japanese.

Despite being considered a hero in Japan, he still struggled to get his ideas accepted by American businesses. The watershed event that would change all that and catapult Deming into the American scene was the 1980 NBC video whitepaper titled, “If Japan Can, Why Can’t We?”

This documentary was about the increasing industrial competition the United States was facing from Japan. As a result of the broadcast, demand for his services increased significantly, and continued throughout the world until his death at the age of 93.

Ford Motor Co. was one of the first American companies to recruit Deming to help jump-start their quality movement. To management’s surprise, Deming’s first order of business was not to talk about quality but about management. Deming stressed that 85% of all problems were management-caused. As Ford implemented more and more Deming principles, quality and profits improved. In a letter to AutoWeek Magazine, Donald Peterson, then-Ford CEO, said, “We are moving toward building a quality culture at Ford and the many changes that have been taking place here have their roots directly in Dr. Deming’s teachings.”

In his book, “Out of Crisis,” Deming’s theory of management presented his famous 14 Points for Management. Management’s failure to plan for the future brings about loss of market share, which results in the loss of job security. He stressed that management must be judged not only by quarterly profits, but by longer-term plans to stay in business, protect their investment, ensure future shareholder dividends, and provide more employment through improved products and services. Deming commented, “Long-term commitment to new learning and new philosophy is required of any management that seeks transformation. The timid and the fainthearted, and the people that expect quick results, are doomed to disappointment.”

Deming left a legacy that lives on in his documented works. He founded the W. Edwards Deming Center for Quality, Productivity and Competitiveness at Columbia Business School to promote operational excellence in business through the development of research, best practices and strategic planning.

His value and benefit to society can be seen in his System of Profound Knowledge, which is the basis for application of his famous 14 Points for Management. Deming taught that by adopting appropriate principles of management, organizations can increase quality and simultaneously reduce costs. The key is to practice continual improvement and think of manufacturing as a system, not as separate initiatives or elements.

Kaoru Ishikawa (1915-1989)

Kaoru Ishikawa was a Japanese icon, university professor and influential quality management innovator. The lifetime work of Ishikawa was extensive and had a profound impact on a worldwide scale. He wrote 647 articles and 31 books, including two that were translated into English: “The Guide to Quality Control” and “What is Total Quality Control? The Japanese Way.”

After World War II, Japan needed to transform its industrial sector, which was then perceived as a producer of cheap toys and poor quality cameras. It was Ishikawa’s ability to mobilize the Japanese government and people toward a specific common goal that was largely responsible for Japan’s quality-improvement initiatives. How was he able to do this? Ishikawa was perceived as wise, insightful and able to get people to focus on the very root of problems.

Ishikawa integrated and expanded the management concepts of Deming and Juran into the Japanese system of quality management. His ability to interpret the primary concepts and translate into a system of improvement techniques and strategies was critical to the Japanese success.

Ishikawa, in conjunction with the Japanese Union of Scientist and Engineers (JUSE), introduced the concept of quality circles. This concept began as an experiment to see what effect the “leading hand” at the worker level could have on quality. It was a natural extension of training that was taking place at other levels of an organization—the top and middle managers having already been trained by Deming and Juran.

Ishikawa believed strongly that all workers must be involved in quality improvement and that teams provided the greatest avenue to participation. People’s ability to offer suggestions to improve processes and products would be maximized by providing training in basic problem solving techniques. A simple concept today, but it was unique at the time.

Quality circles would soon become very popular and form an important link in a company’s total quality management system. At one point there were 1 million active quality circles in Japan, involving more than 10 million people.

He was at the forefront of the Japanese total quality control movement. To reduce confusion between Japanese and the western-style approaches, he called the Japanese method companywide quality control (CWQC). CWQC involved participation of workers from the top to the bottom of the organization and from the start to the finish of the product life cycle; therefore, the approach justifies its name.

Ishikawa became the chairman of the editorial board of the monthly Statistical Quality Control. Becoming so well-known and established, Ishikawa was involved in international standardization activities exposing him to other key concepts and initiatives.

He is well known for developing the concept for the fishbone diagram, also known as the Ishikawa or cause and effect diagram that is used by teams to improve their performance in determining potential root causes of their quality problems. The cause and effect diagram became one of the seven basic quality tools and is utilized thoroughly by teams and recognized around the world for its contribution to improvement.

Perhaps the most dominant leader in JUSE, Ishikawa also served as president of the Japanese Society for Quality Control. He co-founded and served as president of the International Academy for Quality.

His approach to reviewing the quality performance of Japanese companies formed the model Hewlett-Packard used to conduct internal quality reviews. Later, that approach served as a role model for the site visits conducted by the board of examiners for the Malcolm Baldrige National Quality Award.

Upon Ishikawa’s 1989 death, Juran delivered this message: “There is so much to be learned by studying how Dr. Ishikawa managed to accomplish so much during a single lifetime. In my observation, he did so by applying his natural gifts in an exemplary way. He was dedicated to serving society rather than serving himself. His manner was modest, and this elicited the cooperation of others.”

Juran has been called the father of the quality movement. If this is so, then Ishikawa could be called the grandfather of the quality movement. He was wise and openly shared his insight for the benefit of all. He was able to get to the point of a problem, no matter how complex, and offer sound advice. Isn’t this what a benevolent grandfather does?

Armand V. Feigenbaum (1922- )

Armand Vallin (Val) Feigenbaum is an American quality expert, academic, innovator, author, benefactor and businessman. He certainly is considered a quality guru but his life’s work is much broader than that title indicates. He has been a role model for managerial innovation and its implementation.

He was one of the first engineers to be able to speak in the language of management by using financial performance as an indicator of poor quality. Throughout his career, Feigenbaum was able to refine his business theories to demonstrate the economic relationships whereby quality drives commercial performance.

Early practitioners of quality theories and principles focused on inspection and statistical sampling or the use of statistics for process control. Feigenbaum, however, was the first to define a systems engineering approach to quality.

With more than 30 years of hands-on managerial experience and serving as quality champion at General Electric (GE), Feigenbaum was able to develop his system theories. He rose rapidly into increasing levels of responsible positions. While GE’s global quality focus expanded, Feigenbaum made many international contacts with leading businesses in Europe and Japan.

Feigenbaum’s ideas are contained in his now famous book “Total Quality Control,” first published in 1951 under the title “Quality Control: Principles, Practice, and Administration.” This book has been translated into more than a score of languages and the fifth edition was recently released. This book and his related contributions is why Feigenbaum has been called the father of quality management and is widely recognized as a quality guru.

Total quality control, known today as total quality management (TQM), is one of the foundations of modern management and has been widely accepted as a viable operating philosophy across various industry sectors. The integration of previous concepts and methods of quality control into a systematic discipline are what made his work so significant.

Many of his initial contributions were not readily recognized by senior management as critical success factors for sustained business growth. That changed with the NBC documentary, “If Japan Can, Why Can’t We?” which was broadcast on June 24, 1980. Coincidently, the release of the third edition of “Total Quality Control” happened around this time and caused renewed focus on his principles.

Feigenbaum served a pivotal role in the history of the quality movement. Along with Juran and Deming, he established the intellectual framework for quality as a discipline worthy of senior management’s attention. While the quality movement had previously focused on production processes and product quality using statistical tools and analysis methods, the extension of quality into all areas of business, including the service and public sectors, is Feigenbaum’s lasting contribution.

By encouraging a systems approach, he focused organizations on building an understanding of the effects of the flow of their business operations and how one operation had subsequent impact on downstream operations. This systemic linkage among processes was the breakthrough thinking for Japan’s just-in-time management and Ishikawa’s companywide quality control.

Feigenbaum’s second influential concept was financial accountability which he introduced through his emphasis on the impact of the cost of poor quality. He studied the economic effects caused by poor quality performance and characterized quality costs as the costs of prevention, appraisal, as well as internal and external failure. Feigenbaum was the first to introduce the concept of the “hidden” plant. The idea that so much extra work is performed in correcting mistakes that there is a hidden plant within any factory or organization. This work encouraged Philip Crosby’s application of “zero defects” and Feigenbaum’s cost of poor quality indicator as the business measurement standard to assess nonconformance to customer requirements.

Many managers misunderstand one of the more popular sayings attributed to Feigenbaum, “quality is everybody’s job.” What many don’t realize is that Feigenbaum intended this concept to be about establishing accountability for quality. Because quality is everybody’s job, it may become nobody’s job. The idea that quality must be actively managed and have visibility at the highest levels of management.

Feigenbaum continues to expand his theories and principles to a worldwide audience. He co-founded, along with Dr. Kaoru Ishikawa of Japan and Walter Masing of Germany, the International Academy for Quality, an organization that continues to be dedicated to advancing the understanding and use of principles, techniques and tools to benefit society at large.

Along the way, Feigenbaum steered the structure of the Malcolm Baldrige National Quality Award as a member of its inaugural board of overseers, the group that helped create an operational definition of total quality in terms that can be broadly applied throughout society. For his significant contributions, he received the National Medal for Technology and Innovation in September 2008 from President George W. Bush.

When history judges the breakthrough ideas and thought leaders of the 20th century, Feigenbaum will be noted for his enduring contributions. He has earned a unique place in the history of engineering through his definition, advancement and execution of his quality and managerial principles.

Philip B. Crosby (1926-2001)

Philip Bayard Crosby was a legend in the quality movement and was considered one of the leading authorities on the subject. He was a businessman, quality professional, author, trainer, philosopher and management consultant throughout his storied career.

After serving in World War II and Korea, and a brief stint as a podiatrist, Crosby began his career as a quality professional. After working as a test engineer at Crosley Corp and a reliability technician at Bendix Corp., he joined Martin Marietta, now Lockheed Martin, in 1957 as a senior quality engineer. This began the development of his zero defect concepts and his writing and speaking on this topic.

In 1965, Crosby joined International Telephone and Telegraph (ITT) as the vice president of quality. During the 14 years with ITT, he worked with many of the world’s largest industrial and service companies, implementing his pragmatic management philosophy, and found that it worked.

Juran, Deming and Feigenbaum were considered the brains, the academics, of the quality revolution. Where Crosby excelled was in finding a terminology for quality that everyone could understand.

Like Fredrick W. Taylor, also an influencer of Juran, Crosby’s ideas came from his experiences on an assembly line. He focused on zero defects, not unlike the focus of the Six Sigma quality movement (although, even 6 sigma quality produces 3.4 ppm, which is not zero).

Crosby was quick to point out, however, that zero defects is not something that originates on the assembly line. To create a manufacturing process that has zero defects, management must set the tone and atmosphere for employees to emulate. If management does not create a system by which zero defects are clearly the objective, then employees are not to blame when things go astray and result in defects occurring. The benefit for organizations of such a system is a dramatic decrease in wasted resources and time spent producing goods that customer’s do not want.

Crosby published 14 books, all of which were best sellers. His first book, “Quality is Free,” has been credited with beginning a quality revolution in the United States and Europe. It has been translated into 15 languages and sold more than 2.5 million copies. All his books, including his other keynote book, “Quality Without Tears,” were easy to read, so people read them. He popularized Feigenbaum’s idea of the cost of poor quality, that is, figuring out how much it really costs to do things badly.

Crosby showed managers everywhere that doing things wrong made costs skyrocket. More importantly, he showed that management was the root cause of these problems. The book set off a revolution in corporate thinking because it shifted the responsibility for the quality of goods and services from the quality department to the corporate boardroom.

To Crosby, there was no such thing as a quality problem. Systems, individuals, processes or departments cause the problems. This was unique thinking. In a meeting to discuss a problem, a department manager brought up a recent quality problem causing much difficulty. After correcting the manager by saying there was no quality problem, the manager became upset and asked, “then what are we meeting for?” When Crosby said “to discuss the engineering problem,” it spurred much debate but it drove the organization to an enlightened transformation and an improved quality culture.

Crosby left ITT to form his own company, Philip Crosby Associates, where he spread the quality gospel through training and management consulting. He went on to be recognized by corporations around the globe as a guru of quality management, and a business philosopher and innovator who changed the way organizations seek to achieve greater efficiency, reliability and profitability.

In addition to his zero defects concept, Crosby left us with his Four Absolutes of Quality Management, the Five Characteristics of an Eternally Successful Organization, and his 14 Steps to Quality Improvement.

Crosby was a prime mover to promote the expansion of the quality concepts, theories and principles. First as head of quality from the global behemoth ITT and then as one of the most highly respected and sought-after quality management consultants and educators, he played a key role in quality improvement.
Crosby’s legacy continues to live on in better quality in thousands of organizations across the globe. Upon Crosby’s death, a noted quality professional had this tribute, “He was one of the warmest and most focused people I ever had the pleasure to meet and his common-sense approach will be missed.”

It was not our intent to ignore the many notable quality gurus who have given so much to the quality movement. With space limited we had to focus on a small number for this article. However, we should give credit to all the men and women who have toiled to make this society better through their dedicated and tireless work. Q

NOBODY else in the computer industry, or any other industry for that matter, could put on a show like Steve Jobs. His product launches, at which he would stand alone on a black stage and conjure up a “magical” or “incredible” new electronic gadget in front of an awed crowd, were the performances of a master showman. All computers do is fetch and shuffle numbers, he once explained, but do it fast enough and “the results appear to be magic”. He spent his life packaging that magic into elegantly designed, easy to use products.

He had been among the first, back in the 1970s, to see the potential that lay in the idea of selling computers to ordinary people. In those days of green-on-black displays, when floppy discs were still floppy, the notion that computers might soon become ubiquitous seemed fanciful. But Mr Jobs was one of a handful of pioneers who saw what was coming. Crucially, he also had an unusual knack for looking at computers from the outside, as a user, not just from the inside, as an engineer—something he attributed to the experiences of his wayward youth.

Mr Jobs caught the computing bug while growing up in Silicon Valley. As a teenager in the late 1960s he cold-called his idol, Bill Hewlett, and talked his way into a summer job at Hewlett-Packard. But it was only after dropping out of college, travelling to India, becoming a Buddhist and experimenting with psychedelic drugs that Mr Jobs returned to California to co-found Apple, in his parents’ garage, on April Fools’ Day 1976. “A lot of people in our industry haven’t had very diverse experiences,” he once said. “So they don’t have enough dots to connect, and they end up with very linear solutions.” Bill Gates, he suggested, would be “a broader guy if he had dropped acid once or gone off to an ashram when he was younger”.

Dropping out of his college course and attending calligraphy classes instead had, for example, given Mr Jobs an apparently useless love of typography. But support for a variety of fonts was to prove a key feature of the Macintosh, the pioneering mouse-driven, graphical computer that Apple launched in 1984. With its windows, icons and menus, it was sold as “the computer for the rest of us”. Having made a fortune from Apple’s initial success, Mr Jobs expected to sell “zillions” of his new machines. But the Mac was not the mass-market success Mr Jobs had hoped for, and he was ousted from Apple by its board.

Yet this apparently disastrous turn of events turned out to be a blessing: “the best thing that could have ever happened to me”, Mr Jobs later called it. He co-founded a new firm, Pixar, which specialised in computer graphics, and NeXT, another computer-maker. His remarkable second act began in 1996 when Apple, having lost its way, acquired NeXT, and Mr Jobs returned to put its technology at the heart of a new range of Apple products. And the rest is history: Apple launched the iMac, the iPod, the iPhone and the iPad, and (briefly) became the world’s most valuable listed company. “I’m pretty sure none of this would have happened if I hadn’t been fired from Apple,” Mr Jobs said in 2005. When his failing health forced him to step down as Apple’s boss in 2011, he was hailed as the greatest chief executive in history. Oh, and Pixar, his side project, produced a string of hugely successful animated movies.

In retrospect, Mr Jobs was a man ahead of his time during his first stint at Apple. Computing’s early years were dominated by technical types. But his emphasis on design and ease of use gave him the edge later on. Elegance, simplicity and an understanding of other fields came to matter in a world in which computers are fashion items, carried by everyone, that can do almost anything. “Technology alone is not enough,” said Mr Jobs at the end of his speech introducing the iPad, in January 2010. “It’s technology married with liberal arts, married with humanities, that yields the results that make our hearts sing.” It was an unusual statement for the head of a technology firm, but it was vintage Steve Jobs.

His interdisciplinary approach was backed up by an obsessive attention to detail. A carpenter making a fine chest of drawers will not use plywood on the back, even though nobody will see it, he said, and he applied the same approach to his products. “For you to sleep well at night, the aesthetic, the quality, has to be carried all the way through.” He insisted that the first Macintosh should have no internal cooling fan, so that it would be silent—putting user needs above engineering convenience. He called an Apple engineer one weekend with an urgent request: the colour of one letter of an on-screen logo on the iPhone was not quite the right shade of yellow. He often wrote or rewrote the text of Apple’s advertisements himself.

His on-stage persona as a Zen-like mystic notwithstanding, Mr Jobs was an autocratic manager with a fierce temper. But his egomania was largely justified. He eschewed market researchers and focus groups, preferring to trust his own instincts when evaluating potential new products. “A lot of times, people don’t know what they want until you show it to them,” he said. His judgment proved uncannily accurate: by the end of his career the hits far outweighed the misses. Mr Jobs was said by an engineer in the early years of Apple to emit a “reality distortion field”, such were his powers of persuasion. But in the end he changed reality, channelling the magic of computing into products that reshaped music, telecoms and media. The man who said in his youth that he wanted to “put a ding in the universe” did just that.

The product is powered by a 2-cycle gasoline engine and cuts grass or weeds through the use of a spinning black trimmer head containing a spool of filament line. Model number and shaft serial number are located on a label on the shaft halfway between the trimmer head and the engine. The government is set to draw up proposals for international standards evaluating product performance in a bid to help Japanese businesses fare better against price-competitive goods from China and other rivals.

The Ministry of Economy, Trade and Industry is expected to craft its proposals over the next couple of years for submission to international standardization organizations.

The proposals will cover 29 industries where Japan is strong, such as light-emitting diode bulbs, antibacterial textiles and household fuel cells.

Japan has fallen behind the United States and EU countries in the quest for international standardization.

Japan-initiated standards were mainly for products that have smaller markets.

Japanese manufacturers have been put at a disadvantage in the world market because Chinese companies grabbed market share by quickly releasing products similar to theirs, but at much lower prices.

There have been growing calls inside Japan to create a framework that highlights differences in the quality of Japanese and Chinese products.

"Increasingly, it has become important for Japan to get involved in making rules assessing product performance," a ministry official said.

As for LED bulbs, an industry in which Japanese makers have begun mass production, the government is expected to propose to the Geneva-based International Organization for Standardization in fiscal 2012 a method to measure the brightness of a bulb per unit of electric power.

Japan is expected to work with the United States and France to put this plan in motion.

Japanese manufactures use a number of formulas to gauge the energy-saving qualities of their products.

This makes it harder to objectively assess the performance of LED bulbs, which are said to last four to seven times longer than fluorescent lights.

The global market for lighting is estimated at 10 trillion yen ($129.87 billion) a year.

Chinese companies are also rolling out LED bulbs.

The Japanese government fears that, without international performance standards, Chinese makers may market less expensive products, billing them as environmentally friendly based on their own performance tests.

With respect to household fuel cells, used to make electricity and hot water out of gas, the government will propose to the International Electrotechnical Commission, also in Geneva, next fiscal year a method to assess the safety and energy saving feature of the products.

The move is aimed at preparing for the entry of Chinese producers in the future.

Right now, only Japanese companies manufacture household fuel cell on a commercial basis.

The government will also propose to the ISO by the end of the current fiscal year a measure to gauge the performance of sweat absorbing, antibacterial and static protection features of high-function textile.

Other products the government is trying to protect include those involving solar cells or regenerative medical techniques as well as welfare equipment.

International standards at the ISO and IEC are adopted by a majority vote by its member countries.

台灣龍頭醫院台大醫院發生致命疏失，將愛滋病患器官移植到病患上，共有5病患受害。怎麼會發生如此疏失？除了台大醫院在確認捐贈者時檢驗結果時，通報台大醫檢師講的是「reactive」（陽性），接話的器捐移植小組協調師誤聽為「non-reactive」（陰性）外，移植團隊未在手術前二次確認，造成如此遺憾。

男子的器官由台大和成大進行移植手術，成大表示，手術前曾至衛署器捐中心做二度確認，發現台大登錄於該中心資料，捐贈者是愛滋陰性。

談組織出版的能力

昨晚在胡思書店免費取得數本結緣書，都相當不錯。每本書都值得發揮「牢騷力」。先談這本原書是美國品質協會 (ASQ)的品質出版社的翻譯

Finding the Leader in You: A Practical Guide to Expanding Your Leadership Skills by Anton G. Gamarota, Quality press, American Society for Quality, 2004

《成就卓越領導：發現你的領導力》北京：科學技術技能出版社，2006/2008二版

過去近50年來，中華民國品質學會 CSQ在出版方面，不管是《月刊》和叢書，

都採取設立出版委員會的方式應付，初期可以，然而現在狀況令人很尷尬了。

日本過去60年，「品質與生產力」這領域，至少有數家常青的出版社出版月刊和叢書：科技連盟 JUSE、規格協會、日本能率協會。他們還有專門的出版社負責翻譯日本人的著作成英文並行銷全球的，譬如說 3A 出版公司。

台灣的對應單位CSQ和CPC 等，都各有奇怪的發展，不過成績馬虎。

美國的ASQ 的會員月刊Quality Progress 等一向還可以。不過，到了80年代，他們發現在出版上，除了老牌的管理協會AMA之外，遠落後日本。所以私立的如Productivity Press 和 ASQ的 Quality Press 成立了。前者譯介不少日本的東西，而後者訂下的出版規格是：只要內容品質還可以，就出版──美國的出版品的標準已建立，包括幾乎每本書一定有索引。

這20多年來，Quality Press 可能已出版數百本書了。在我這種挑的人，似乎沒一本是讓我眼睛一亮的佳作，然而它保持既定的方針出版，現在「蔚為大觀」。

我覺得慚愧的是，與林公孚和戴久永老師當過CSQ的出版委員 (90年代初)，不過成績幾乎是零。

這系列之文是是記與朋友的一些交往。

這幾天，慧玲報告Mike 的狀況；Ken 來訪，我們其實談過許多主題，少數如賴世和英譯/研究圓仁的《入唐求法[巡禮記》想成blog。

最近似乎決定不出版《領導與學習》。我最覺得對不起的是高雄的好友，他還用認購20本來鼓勵我。另外下不了來的是，Joyce Orsini 教授的序文早就送到。所以現在還在努力…..

今午林公來電，也談了許多事，就順便記在這兒。

林公說我有沒興趣取得品質學會的個人獎？這實在是很尷尬的問題：因為它要給的「他」(即，我) ，應該是1982-85年的我才對。我建議林公，務必把各種獎，送到得講者可以有激勵作用的年青的業者手中。我跟他說ASQ 的榮譽會員才是世界肯定 (昔日Kondo/ Kano 等教授得此，在日本都辦慶祝會) 。而日本的戴明個人獎60年和「日經品質文獻賞」更日本的寶庫。

也許10年前，學會關秘書長說學會要推我去選「國家品質獎」，我都覺得或許晚了10年。哈哈。推掉的藉口是王晃三等等都沒取得，我那敢躐等？

我與林公談的真正重點是，中華民國的品質學會比中國的任一同業都早20年起步，如今比不上什麼北京的、上海、福建、廣州的學會 (網路上的實力對比，真是另一笑話！)。這充分反映台灣的空洞化以及組織人才的凋零。哀哉，吾輩心死矣。

中華電信光世代廣告騙很大

〔記者施曉光、楊久瑩、王憶紅、陳炳宏／綜合報導〕中華電信推出50M/5M光世代「優惠」方案，廣告強調網速超快，可以盡情飆網，但國民黨立委羅淑蕾最近接獲民眾檢舉，指控該廣告騙很大，實際測速不到20M/2M，立委實測立法院號稱100M的速度，也連10M都不到。

中華電信昨天解釋，原因可能是舊型集線器限制速率，測速網頁不準，或電腦未升級，像ＸＰ等舊版作業系統都會影響速率。

羅淑蕾質疑，中華電信壟斷經營，卻不願意提升品質，主管機關應該要求電信業者保證頻寬，中華電信要立即改進上網龜速或降價還錢，她並要求ＮＣＣ及公平會立即調查中華電信的不實廣告行為。

中華電︰客戶端軟硬體有影響

中華電信數據分公司副總經理鍾福貴表示，50M的用法，是國際慣用用詞，一般用戶資料傳輸速率因牽涉到網際網路、網路互連與客戶端軟硬體設備環境等因素而有影響，且中華電信網站、合約文件都有說明，立委說中華電信廣告不實，「這樣的指責，太嚴厲了」。

不過，行政院消保會督導組組長劉清芳表示，ＮＣＣ應該揭露各家業者平均上網速率，各家電信業者也應該明白告知消費者，家中配備可以達到的上網速度極限。

ＮＣＣ營運管理處簡任技正吳銘仁則說，已要求中華電信幫消費者裝機時，要當場測試上網速度，消費者接受才確認申裝，且ＮＣＣ每個月都將抽測上網品質，並適當揭露資訊。

消基會董事長蘇錦霞昨指出，中華電信對外宣稱與實際速度不符的問題確實存在，消基會已收到不少投訴案例。中華電信應跟消費者說清楚，究竟買50M光世代跟20M有何差別？並確實告知線路、距離及相關配備可能造成的影響，才不至於給消費者過高期待，進而引發消費糾紛。

消基會與經濟部標檢局抽檢市售抗UV防曬衣的品質安全及抗UV效果發現，知名品牌的防曬衣號稱防曬，卻連最基本的防曬國家基本標準抗UV係數UPF15都未達，另一知名品牌的紫外線係數膨風，防曬係數僅有UPF23.2（C級），卻標示為最高等級的A級，提醒消費者注意。（記者廖振輝攝）

〔記者楊久瑩／台北報導〕豔陽高照，抗紫外線（UV）的防曬衣是入夏必備商品，不過，消費者文教基金會與經濟部標檢局抽檢結果卻發現，有知名品牌的防曬衣號稱防曬，卻連最基本的防曬國家基本標準抗UV係數UPF15都未達；也有知名品牌的紫外線係數膨風，防曬係數僅有UPF23.2（C級），卻標示為最高等級的A級。

抽驗百貨、專賣店及購物頻道12件樣品

為了解市售抗UV防曬衣的品質安全及抗UV效果，標檢局及消基會，於今年5月在大台北地區百貨公司、專賣店及電視購物頻道採購12件防曬衣樣品，針對游離甲醛、防紫外線性能、服飾標示基準進行查核。

檢測結果發現，「墾趣生活」女彈性棉快乾抗UV短上衣標示與實際效果不符、「無印良品」女開襟衫則不符國家基本抗UV係數標準，不得標示抗UV；經濟部中部辦公室將行文要求這兩家業者下架、改善，否則將處以3萬到30萬元不等的罰鍰，並得連續罰至改善為止。

2知名品牌遭點名　業者：尚未收到公文

不過，包括墾趣生活及無印良品兩品牌進口業者翔雁國際及台灣無印良品公司都表示，目前尚未收到經濟部公文，由於防曬衣的製造商分別來自美國及日本，已緊急與國外製造商連繫，確認產品的防曬狀況，如確如消基會及標檢局的抽檢結果將配合下架改正，如標示無誤也會向標檢局反應。

消基會董事長蘇錦霞指出，這次檢測的樣品價格均介於新台幣299至1598元之間，但價格高低卻無關抗UV效果。此次抽檢，在「游離甲醛」部分全數符合國家標準，不過，防紫外線（UV）性能則有2件不符合國家標準，依國家標準防紫外線係數（UPF）分為3級，A級（40~50、50+）、B級（25~39）、C級（15~24）。織品衣物至少須防紫外線係數（UPF）15以上，才符合抗紫外線標準，且UPF值越高防曬效果越佳。

民眾購買前應注意完整中文標示及觸感

標檢局第二組組長張文彬指出，檢測的樣品中「墾趣生活」女彈性棉快乾抗UV短上衣，標示的防紫外線係數為50+，與實際檢測值UPF23.2不符，未達到其標示上宣稱的A級標準；另外「無印良品」女開襟衫，防紫外線係數(UPF)實際檢測值只有8，不符合至少C級的規定。

另外「中文標示」：檢查有3件樣品不符合「服飾標示基準」之規定；2件實際纖維成分與標示不符根據「服飾標示基準」的規定。

蘇錦霞指出，纖維成分及產地未以中文標示的商品已違反商品標示法規定，業者必須限期改正，屆期不改正將處新台幣2萬元以上20萬元以下罰鍰；至於有2件樣品，其防紫外線(UV)性能與標示或宣稱不符、另有2件實際纖維成分與標示不符，落差超出3％許可差的規定，也將要求業者限期改正，屆期不改正者，將處新台幣3萬元以上30萬元以下罰鍰，並得按次連續處罰至改正為止；如情節重大，還得令其停止營業六個月以下或歇業。

標檢局及消基會呼籲，消費者選購時應注意檢視中文標示是否完整，包括製造商或進口商的名稱、電話及地址、尺寸或尺碼、生產國別（產品主要製程地之生產國別）、纖維成分及洗燙處理方法等；其次，挑選防曬衣時，除以手觸摸檢視布料質感外，可以嗅覺檢查，如有刺鼻異味請勿購買。在穿著前最好先以清水洗滌過，以減少吸附在其表面之化學物質含量。

署立雙合醫院主治醫師許浚翃昨則建議，防曬除了美白，也是為了健康保護皮膚，完整的防曬措施除了塗抹防曬乳、隔離霜外，還需加上抗UV的太陽眼鏡、洋傘、寬邊帽子及抗紫外線的衣物，並且避免在上午10點至下午3點陽光最強烈的時段外出，至於防曬乳原則上防曬係數SPF愈高，防曬效果愈好，且每隔2到3小時就應塗抹一次。

Three former Labour cabinet ministers were suspended earlier this week following an undercover investigation by British media.

Former Defense Minister Geoff Hoon, Stephen Byers and Patricia Hewitt were caught on film during a meeting with a reporter who was posing as a lobbyist. All three appeared ready to influence government policy or provide other lobby work in exchange for cash. Byers, the former transport secretary, even described himself as a cab for hire for up to 5,000 pounds a day. The incident constitutes a severe setback for Gordon Brown's Labour Party and comes just ahead of the general election in May. Neil King asked Phil Webster, the election editor of The Times newspaper, just how damaging this fresh scandal might prove to Labour's campaign and UK politics in general.

網友酸「被動車碰了瓷」

〔國際新聞中心／綜合報導〕北京故宮7月31日向媒體證實，研究人員日前在對文物進行「無損分析測試」時，不慎造成國家一級文物、宋代哥窯代表作品「青釉葵瓣口盤」損壞。由於中國日前才因浙江溫州動車追撞事件造成極大民怨，有網友憤怒指控，「故宮，你被動車碰了瓷？」聲稱這件古物是被工作人員給摔碎的！

北京故宮表示，7月4日上午約10時，故宮古陶瓷檢測研究實驗室科研人員，從古器物部調出宋代哥窯青釉葵瓣口盤，進行無損分析測試，科研人員在操作儀器時「不慎失誤」，導致儀器內的樣品台上升距離過大，使瓷器受到擠壓而損壞。事發後，科研人員立即停止儀器運行，保護現場，院相關部門負責人和主管院領導立即趕到現場處理。

哥窯瓷土脈微紫、質薄，有油灰色、米色、粉青色3種瓷釉彩，表面佈滿裂紋。因為土質含鐵量較高，燒胚時發生氧化，瓷器胚呈紫黑鐵色，瓷器沒有塗釉的底部顯現瓷胚本來的鐵色，叫「鐵足」，而釉彩較薄的口部呈紫色，叫「紫口」。

北京故宮聲稱，鑑於宋代5大名窯的學術研究中長期存在諸多問題，故宮近年來結合傳統研究方法和現代科技，逐步展開檢測、研究。從去年開始，他們利用這台「無損監測儀器」順利完成50餘件陶瓷文物的分析測試，但青釉葵瓣口盤意外損壞後，測試工作已經暫停。

據了解，故宮文物受損的消息，是因為有網友在微博（微網誌）上發文才曝光。一名署名「龍燦」的網友在新浪微博上表示：「故宮又出大事了！」聲稱故宮器物部手續不全，將國家一級品宋代哥窯瓷器出庫，不料卻被工作人員摔碎，而故宮一級品才1106件。微博很快被轉發過萬，不少網友表示惋惜，「極其珍稀的宋代哥窯再減一員，默哀」。

網友並指出故宮有瞞報嫌疑，「如果沒有『龍燦』的爆料，故宮是不是打算瞞過去？」

哥窯為宋代5大名窯之一，受損的「青釉葵瓣口盤」呈6瓣葵花式，通體施青灰色釉，釉面開細碎片紋，圈足露胎處呈黑褐色，造型優雅、大方，線條富於變化，為哥窯代表作品。

哥窯又名哥哥窯、琉田窯，為宋代「汝、官、哥、鈞、定」5大名窯之一，以紋片著稱。相傳宋代浙江龍泉章氏兄弟各主窯事，哥哥章生一在龍泉琉田創建的瓷窯稱為哥窯，弟弟章生二則稱弟窯。

由於哥釉瓷細緻精美，此後各朝代都有仿造，尤其是清代，還出現仿哥釉瓷的高潮。到清朝後期，哥釉明顯不如前期，顏色越來越深，開片越來越細碎，釉面甚至出現凹凸不平的疙瘩釉，胎質也變得疏鬆。（國際新聞中心）
